When you’re ready to leave rehab, you will have completed an intense and challenging journey. You will have worked hard to overcome your addiction, and you will have made progress in your recovery. The drug rehab discharge process is designed to help you transition back to your life outside of rehab and to set you up for success in your continued recovery.

The rehab discharge process can be challenging to navigate for those who are not familiar with it. At Red Oak Recovery®, we understand this and are here to help. Understand the Rehab Discharge Process

When you are discharged from rehab, you will receive a set of discharge papers. These papers will include information about your treatment, your progress in treatment, and your next steps in recovery. It is important that you take the time to read and understand these papers, as they will contain important information about your continued care.

Final Appointments

Before you graduate from rehab, you will have a final appointment with your therapist and addiction counselor. This appointment is an opportunity to discuss your next steps in recovery.

Aftercare Planning

After you graduate from rehab, you will meet with your case manager or discharge planner to develop an aftercare plan. This plan will outline your continued care after you leave rehab. It will include information about your follow-up appointments, support system, and recovery goals.

Transitional Living

Your aftercare plan may include a stay in a transitional living setting. Here, you will continue to receive support from the Red Oak community in a safe environment. During your stay in transitional living, you will begin to reintegrate into your normal life without the triggers that may stem from your home.

Steps After Addiction Treatment to Maintain Sobriety

After you have completed an addiction treatment program, there are a few important steps after addiction treatment you need to take to maintain your sobriety and support your continued recovery.

  • Follow your aftercare plan: This plan will outline your continued care after you leave rehab. It is important that you follow this plan and that you make all of your follow-up appointments.
  • Stay connected to your support system: It is important that you stay connected to your support system after you leave rehab. This could include your family, friends, therapist, and addiction counselor. These people can provide you with support and guidance as you navigate your recovery.
  • Avoid high-risk situations: Certain situations can trigger your addiction, such as being around people who use drugs or alcohol, being in places where drugs or alcohol are being used, or being in stressful or overwhelming situations. Avoiding these high-risk situations can help you maintain your sobriety.
  • Practice self-care: It’s vital to practice self-care to maintain your sobriety. This includes eating a healthy diet, exercising regularly, getting enough sleep, and managing stress.
