What Is a Rehab Discharge Plan?

  • Identify your support system – Who will you rely on for recovery support? Consider family, friends, a therapist, or a sober support group. Identifying your support system can help you create a plan for how to stay connected to them and get help when you need it.
  • Make a list of your triggers – What are the things that trigger your urge to use drugs or drink alcohol? This may include certain people, places, or things. Making a list of your triggers can help you avoid them and stay on track in recovery.
  • Develop a “break the glass” plan – What will you do if you feel like you’re about to relapse? This may include calling your sponsor, going to a meeting, or talking to your therapist. Having a “break the glass” plan in case of emergencies can help you avoid relapse and stay on the path to recovery.
